<br />HF IT ORDAINTYI), That it shall be the duty of all physicians and midwives to register
<br />their names wdth the TTealth Officer of the city.
<br />2. That every Physician, midwife or any other person in attendance upon t}je birth
<br />of a child shall send to the Board of 71ealth within three (3) days after t, -,Ie births

<br />such child a certificate setting; forth, as far as can be ascertained, the sex and cc
<br />of child, the place of birth and the date of birth..
<br />3. PFIIALTY In case any physician or midwife or any other person in attendance
<br />upon the birth of a eh4d, shall refuse or neglect to furnish such certificate as
<br />aforesaid he or she shal.1 forfeit and pay the eurn of five dollars (.;`'S) for eac},.
<br />offense. This ordinance shall be in force on and after its passage.
