205 <br /> <br />St. Charles Avenue and-from the Hospital .access east. <br /> <br /> 7. Voluntary agreement On the part of Martha Jefferson <br />Hospital (the "Hospital"') to expand the current special <br />permit boundary so that 'the amended special :permit site <br />includes all of the parcels between former Taylor Street and <br />Sycamore Street that front~on Locust Avenue (Parcels 233, <br />232, 23!~ 230, 229, 228 andli227' on City Tax Map 53). Said <br />agreement is to be evidenced by a letter or written consent <br />statement in behalf of the Hospital by its Board of Directors <br />delivered to the Clerk of Council, which shall be attached to <br />and become a part of this special use permit. <br /> <br /> Inclusion of the Locust avenue properties in this <br />fashion is necessary to protect nearby residential properties <br />from adverse affects arising from the expansion and operat.ion <br />of the Hospital. Any changes in the use of the above parcels <br />after May 6, 1991, which cause significant changes in the use <br />or the .exterior appearance of those parcels and improvements <br />thereon shall be subject t© approval of the City through the <br />special use permit process. <br /> <br />Conditions from May 2, 1988'~pecial use permit <br /> <br /> 1. Administrative apprOval of a final detailed site <br />plan Which shall include, among other things, the following: <br /> <br /> a) Location of all minimum building setback lines~ <br /> b) Correct site elevations and building height <br /> elevations; <br /> c) A detailed sign package in compliance with the <br />City Code for all proposed signage; <br /> d) Storm water run-off calculations and storm water <br />detention calculations~ <br /> e) Information-pertaining to the location and sound <br />buffering of proposed mechanical equipment; <br /> f} Further details on specific landscaping plant <br />materials in a final landscaping plan~ <br /> g) A signature panel; <br /> h) A grading and soil erosion/sediment control plan <br /> filed separately for review and approval prior to final site <br /> plan approval~ and <br /> i) A plat submitted separately showing the combined <br />parcels in the special use permit application (including <br />those parcels of land on which the existing hospital is <br />located), as a single lot once the street closings occur. <br /> <br /> 2. That intersection/corner radius improvements be made <br />at the following intersections: <br /> <br /> a) Lexington Avenue and East High Street - <br />Northeast corner radius increase and alignment of street curb <br />edge toward Locust Avenue. <br /> b) Taylor Street and Lexington Avenue - Southeast <br />corner radius increase. <br /> c) Locust Avenue and Sycamore Street - Northwest <br />and southwest corner radius increases to 15' along with <br />curbing. <br /> <br /> 3o The provision by the applicants of a pedestrian walk <br />signal system at the intersection of Locust Avenue and East <br />High Street. <br /> <br /> 4. The design of the Sycamore Street parking access to <br />discourage entry and exit from the LexingtonAvenue side and <br />encourage access by way of the Locust Avenue side only. <br /> <br /> 5o The design and development of a storm water <br />detention system as part of the expansion project including <br />the Phase I Parking Decks, <br /> <br /> 6. The development of the additions to the Martha <br /> <br /> <br />
