Death benefit for certain employees. <br />Every employee initially employed by the city on or after <br />January 1, 1978, and every employee reemployed by the city on or <br />after January 1, 1978, whether any prior period of creditable <br />service was served before January 1, 1978, shall upon service <br />retirement or upon attainment- e.€- e3-- retri* - tt-e -- -can <br />disability retirement or while still employed, become eligible for <br />a post- retirement lump sum death benefit in lieu of being covered <br />by group life insurance following such event, in the amount of two <br />thettsand- bruirwired- eleiiar -i � &O-. five thousand dollars <br />($5,000.00). On July 1, 1994, and on that date in each succeeding <br />ear the amount of the lum+ sum death benefit shall be ad'usted b <br />the same percentage change in the Consumer Price Index during the <br />preceding year. Such benefit shall be payable upon the death of <br />such retired employee to a beneficiary as designated in writing by <br />the employee on the application for retirement benefit form <br />supplied by the city, or to the estate of such employee if no such <br />beneficiary designation has been made. <br />Approved by Council <br />December 21, 1992 <br />k of City Council <br />
