shall be available for City inspection. Copies of responses from the regulatory <br />agencies to Grantee shall likewise be made available. • <br />8.03 Annual Report. On or before July 1, including the year in which the <br />Franchise becomes effective, Grantee shall file with the City a copy of the <br />following information regarding the System, in such a format as can be <br />specifically allocated to the Franchise Area that is legally included within the City <br />of Charlottesville: <br />A. A financial statement including an income statement, statement of <br />operating expenses in detail, a cash flow statement and a balance sheet <br />prepared in accordance with generally accepted accounting principles and <br />certified by a financial representative of Grantee. Grantee's SEC Form 10- <br />K shall suffice for such purposes. <br />B. A description of the Services then being offered at the end of the <br />fiscal year. <br />C. A current copy of the Subscriber Service information required in <br />accordance with Section 9.05 hereof. <br />D. A current list of names and addresses of each principal. For the <br />purposes of this requirement the term "principal" means any Person, firm, <br />corporation, partnership or joint venture, or other entity who or which owns • <br />or controls five percent (5%) or more of the voting stock (or any equivalent <br />interest of a partnership or joint venture of Grantee). <br />E. A compilation summarizing the Complaints received during the <br />reported year, by category, and a discussion of any unresolved <br />Complaints. <br />8.04 Additional Reports. Grantee shall prepare and furnish to the City, within <br />reasonable time, such additional reports with respect to its operation, affairs, <br />transactions or property, which are reasonably necessary for the administration <br />and enforcement of this Franchise. <br />8.05 Maps. Grantee shall maintain on file with the City at all times a current <br />map or set of maps drawn to scale showing the System and all equipment <br />installed or in place in Streets and other public places. <br />8.06 Periodic Inspection. The City and Grantee shall, at the request of the City, <br />undertake an inspection of the System to ascertain the System performance at <br />randomly selected Subscriber Drops selected by the City. The request for such <br />an inspection will be made on the basis of Complaints received or other evidence <br />indicating an unresolved controversy or problem. <br />22 <br />
