(7) CUSTODIAL SERVICES: The Boys & Girls Club will assume full responsibility for all <br />• custodial services for the leased premises. Such service shall include the regular clean-up and <br />maintenance of the vending area, exterior front entrance, and all portions of the interior of the <br />Smith Center used by the Boys & Girls Club. Restrooms shall be cleaned and restocked with <br />necessary supplies on a daily basis. The Boys & Girls Club shall also be responsible for dusting, <br />high -cleaning, vacuuming, carpet cleaning, mopping, and stripping, waxing, and buffing of all <br />floors. All custodial services shall be performed as often as necessary to keep all leased premises <br />in a clean, sanitary and safe condition. <br />(8) FACILITY MAINTENANCE AND REPAIRS: The City will assume responsibility for <br />facility maintenance and repairs, i.e., plumbing, replacement of light bulbs, building leaks, <br />elevator maintenance, broken glass windows and doors, HVAC and utilities. All requests for <br />facility maintenance and repair services shall be made to the Department of Parks and Recreation <br />Director or City Recreation Center Supervisor. Painting and minor repairs will be the <br />responsibility of the Boys & Girls Club. Paint colors must be approved in advance by the <br />Department of Parks and Recreation Chief of Operations or the City Recreation Centers <br />Supervisor. <br />(9) TRASH COLLECTION: Daily trash collection will be provided by the City's Recreation <br />Department Maintenance staff. The Boys and Girls Club will be responsible for collecting all <br />trash in large plastic bags and placing them in a central designated area for daily pick-up. The <br />Boys and Girls Club will be responsible for bulk item disposal in accordance with requirements <br />of the City Code. Trash not collected and placed in the designated area will not be collected by <br />• the City staff. <br />(10) FACILITY INSPECTION: <br />(a) At any time during the term of this Agreement the Superintendent for Building Maintenance <br />for the City's Department of Public Works may inspect the leased premises to determine <br />Compliance with this Agreement. A `Building Inspection Form", a copy of which is attached <br />hereto as Attachment A, shall be completed during the inspection as a record of the condition of <br />the leased premises. A representative of the Boys & Girls Club will be entitled to be present <br />during the inspection and, upon request, will be provided with a copy of the completed Building <br />Inspection Form. The City will notify the Boys & Girls Club in writing of any noncompliance <br />with this Agreement, and require that the noncompliance be corrected within a specified time. <br />(b) The City may suspend the use of the premises by the Boys & Girls Club if the noncompliance <br />is not corrected within the specified time, or if the Boys & Girls Club is cited for violations of <br />this Agreement three (3) or more times within any twelve (12) month period. Notice of the <br />suspension shall be sent by registered or certified mail to the Executive Director of the. Boys & <br />Girls Club. The City may allow the Boys & Girls Club to resume using the facility upon <br />submission by the Boys & Girls Club of a written plan which includes measures to address any <br />existing violations and to prevent future violations. Failure to submit a written plan within <br />fourteen (14) days of the notice of suspension shall be grounds for immediate termination of this <br />Agreement. <br />0 2 <br />
